A modest proposal for research.

چکیده
Complexity is conducive to dependence on specialists on the one hand, and mistrust of them on the other. If you read in the Bulletin, “A dendrogram of sequence-relatedness was constructed by the method of the VPI maximumlikelihood bootstrap consensus tree (PUZZLE, version 4.0)” (1), and this is not in your area of specialization, you might respect it for its apparent professionalism, resent it for its obscurity, or just tune it out. The usefulness of such information must be judged in relation to what the researchers were trying to do. In this case they were trying to calculate the implications of three cases of paralysis caused by wild poliovirus which had occurred in Roma children in the eastern harbour city of Bourgas, in Bulgaria, 10 years after the last case had been reported. In the context of the polio eradication campaign, the objective of the study was valid and it came to a plausible conclusion, so the Bulletin’s editorial committee was inclined to trust the authors, together with the peer reviewers whose verdict was that the methods were OK. Trust is indispensable but so is wariness, as Uwe Reinhardt & Tsung-mei Cheng pointed out in their review of The world health report 2000 (2). They objected that “to see what was actually done, one must plough through the cryptic commentary that accompanies the tables in the Annex or dig up and read sundry sources cited in the references”. Having done that, they suspected that the measures were flawed and the findings invalid. More fundamentally they questioned the value of the attempt itself, which was to rank the national health systems of 191 countries in order

Health Services Research Spending and Healthcare System Impact; Comment on “Public Spending on Health Service and Policy Research in Canada, the United Kingdom, and the United States: A Modest Proposal”
The challenges associated with translating health services and policy research (HSPR) evidence into practice are many and long-standing. Indeed, those challenges have themselves spawned new areas of research, including knowledge translation and implementation science. These sub-disciplines have increased our understanding of the critical success factors associated with the uptake of research ev...
متن کاملMaking Research Matter; Comment on “Public Spending on Health Service and Policy Research in Canada, the United Kingdom, and the United States: A Modest Proposal”
We offer a UK-based commentary on the recent “Perspective” published in IJHPM by Thakkar and Sullivan. We are sympathetic to the authors’ call for increased funding for health service and policy research (HSPR). However, we point out that increasing that investment – in any of the three countries they compare: Canada, the United States and the United Kingdom– will ipso facto not necessarily lea...

Health services and policy research (HSPR) represent a multidisciplinary field which integrates knowledge from health economics, health policy, health technology assessment, epidemiology, political science among other fields, to evaluate decisions in health service delivery. Health service decisions are informed by evidence at the clinical, organizational, and policy level, levels with distinct...
متن کاملPublic Spending on Health Services and Policy Research in Canada: A Reflection on Thakkar and Sullivan; Comment on “Public Spending on Health Service and Policy Research in Canada, the United Kingdom, and the United States: A Modest Proposal”
Vidhi Thakkar and Terrence Sullivan have done a careful and thought-provoking job in trying to establish comparable estimates of public spending on health services and policy research (HSPR) in Canada, the United Kingdom and the United States. Their main recommendation is a call for an international collaboration to develop common terms and categories of HSPR. This paper raises two additional q...
متن کاملAnimal rights v animal research: a modest proposal.
The practical problem of assuaging the opponents of animal research may be solved without formally addressing (or resolving) the underlying ethical questions of the debate. Specifically, a peaceful boycott of the "fruits" of animal research may lead to a wider cessation of such research, than, say, vocal or even violent protest. To assist those who might wish to participate in such a boycott- a...
متن کاملA modest proposal for a longitudinal study of dementia prevention (with apologies to Jonathan Swift, 1729).
Many studies have documented the role of risk and protective factors for late life dementing illnesses, particularly Alzheimer's disease. A "Systematic Review" from the US Agency for Healthcare Research and Quality and the National Institute on Aging concluded that because the overall quality of evidence was low, recommendations for public health could not be made. In order to gain evidence for...
